First Visit:
When you arrive for your first visit we ask that you come 15 minutes early and be prepared to complete all insurance and health information forms that will allow us to begin your dental treatment. We will ask you to fill out several forms that will get you acquainted with our office.
We will ask for a photo I.D. at your first visit along with your dental insurance card. We will also take a current photo for future identification as required by the new Federal Red Flag Laws.
If you have current, diagnostic X-rays from a previous dental practice, please bring them with you to your dental appointment. Otherwise, we will take a complete set of X-rays so that we can do a complete and thorough exam.
Your initial exam will last approximately 1 1/2 hours. On the first visit we will take a complete series of X-rays, Oral hygiene instructions will be provided along with suggestions to help you care for your teeth. In most cases, we will also clean your teeth on this visit and provide a complete oral evaluation that will outline your existing dental needs and propose treatment.
Appointments:
Our appointments are scheduled to respect your time. We reserve a specific time for your care and we make every effort to see you as scheduled. We appreciate your promptness. We reserve the right to reschedule an appointment if a patient is more than 10 minutes late. If you do need to change an appointment, we ask that you give at least a 24-hour notice so that another patient can benefit from the appointment time.
Insurance:
We file dental insurance as a courtesy and are glad to assist you with your dental insurance questions. To help us assist you in obtaining your maximum benefit, please bring your dental insurance card or complete insurance claim form with phone number to your visit. Verification of insurance is not a guarantee of payment. You are responsible for any service provided that is denied or not paid by your insurance company. Most plans only cover a portion of the dental fee, which means you will be responsible for your deductible and the portion we estimate not covered by your plan at the time of your visit.
Secondary Insurance:
We do not accept assignment of benefits on secondary insurance. If you have secondary insurance, we will file your primary insurance for you; however, you are asked to pay anything not covered by your primary insurance at your appointment. We will provide you with necessary forms so that you can submit the secondary insurance for reimbursement.
Billing:
We estimate your portion on the date of service based on information given to us by your insurance carrier. However, sometimes there is a need to send you a statement for the portion insurance has not paid.
